Leaving hospital after stroke. Where will I go after my stroke?
The experience of a stroke is an extremely traumatic and confusing time for you and for your family. You will wonder where you will go after you leave hospital. Although individual circumstances vary a great deal and you should receive support and advice on your discharge, there are four general pathways that may occur. You may:
1. Go directly home from the acute stroke unit or medical ward
2. Be admitted to a specialised rehabilitation unit
3. Be transferred to low or high residential care (hostel and nursing home care)
4. Receive palliative care
